Our Commitment to Your Privacy
San Diego Cardiac Center Medical Group, Inc. (“we,” “us,” “our”) is dedicated to protecting the privacy of your health information. This Notice describes how we may use and disclose your protected health information (“PHI”), and your rights regarding that information under the Health Insurance Portability and Accountability Act (“HIPAA”).

- Treatment: Sharing information with physicians, nurses, technicians, and other personnel to provide you care.
- Payment: Billing your insurance or other third-party payer for services you receive.
- Health Care Operations: Quality assessment; staff training; case management; accreditation and licensing; business management.
- Appointment Reminders and Health-Related Benefits: Reminding you of upcoming visits; informing you about treatment alternatives, health-related products, or services.
- Public Health and Safety: Reporting communicable diseases, child or elder abuse, and threats to public health or safety.
- Legal Requirements: Complying with court orders, subpoenas, or other legal processes.
- Organ Donation: Assisting with organ, eye, or tissue procurement and transplantation.
- Research: Under strict protocols, for approved research that meets all HIPAA requirements.
Uses and Disclosures Requiring Your Written Authorization
Except for the purposes listed above, we will obtain your written authorization before using or disclosing your PHI. You may revoke an authorization in writing at any time, except to the extent we have already acted upon it.
Your Rights Regarding PHI
You have the following rights concerning the PHI we maintain about you:
- Right to Inspect and Copy: You may request access to your medical record and billing records in paper or electronic form.
- Right to Amend: If you believe your record contains an error or omission, you may request a correction.
- Right to an Accounting of Disclosures: You can request a list of disclosures we have made of your PHI for purposes other than treatment, payment, and health care operations.
- Right to Request Restrictions: You may request limits on how we use or disclose your PHI. We are not required to agree but will comply if feasible.
- Right to Confidential Communications: You may ask that we communicate with you in a certain way or at a certain location.
- Right to a Paper Copy of This Notice: You may request a paper copy of this Notice at any time.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with:
- San Diego Cardiac Center Privacy Officer
- U.S. Department of Health and Human Services, Office for Civil Rights
We will not retaliate against you for filing a complaint.
Changes to This Notice
We reserve the right to change this Notice at any time. The revised Notice will apply to all PHI we maintain. A copy of the current Notice will always be posted in our office and on our website.
