

Your First Visit
What to Bring
Prior to your first visit, you will receive a welcome package from our staff. It is very important to complete all of the enclosed forms prior to your appointment.
Please bring the following:
- all your medications
- medical insurance card
- applicable co-pay
- completed registration forms
You are encouraged to bring your medial questions and concerns to discuss them with your cardiologist. Family members and friends are always welcomed.
What to Expect
The first appointment is designed for you to meet with the cardiologist to review your medical history and current cardiac symptom(s) and/or condition(s). The following activities will occur at your first appointment:
- A receptionist will greet you, obtain your completed new patient forms and applicable co-pay, verify insurance and copy your insurance card.
- A nurse will review your medications with you, measure your weight, blood pressure and pulse and obtain an electrocardiogram, as necessary.
- Your cardiologist will talk with you about your medical history, cardiac symptoms and/or condition and may recommend preventative management or diagnostic tests to collect further cardiac-related information.
- If you were referred by your primary care doctor, a consultation report will be provided to them.
- As necessary, a follow-up appointment at San Diego Cardiac Center will be scheduled by a receptionist.
Health and Clinical History - Please bring this form already completed, as this is vital baseline information for your cardiologist to review at the beginning of your first appointment.
